Application for Independent Contractor

Contractor applicants to drive a CDL vehicle must provide employer and job information below for the past 10 years. Please list your most recent employers first.

As an independent contractor for Spirit Miller Trucking, LLC, I consent to a prequalification drug screen as required in FMSCR 49 CFR Part 382 and company policy. I understand that if contracted by Spirit Miller, I am subject to in-service drug/alcohol testing as required by those regulations. I agree to hold Spirit Miller harmless in the administration of these federal requirements. Cost for prequalification testing shall be at the expense of the independent contractor.

I authorize Spirit Miller trucking, LLC to make such investigations and inquiries of my personal, employment, financial, judicial, or medical history and all other related matters as maybe necessary in arriving at a qualification decision. I further authorize all previous employers and lessees to release and forward all information on my alcohol and controlled substances testing/training record the Spirit Miller pursuant to section 391–23 of the Federal motor carrier safety regulations. I hereby release employers, institutions from all liability in responding to inquiries in connection with my application.

In the event of qualification, I understand that fall from misleading information given in my application or interviews may result in disqualification.

This certifies that this application was completed by me, and that all entries on it are true and complete to the best of my knowledge, and in no way obligates spirit Miller to approve me as an independent contractor.

I understand upon the activation from Spirit Miller trucking, LLC my contractor leasing status will be entered and turned into the DAC services system and is kept in their record.

The permanent record will include, but is not limited to, drivers name, Social Security number, driver license number, state of issuance, at the time of leasing, period of service, number of chargeable accidents, eligibility for re-contracting, reason for leaving, equipment operated, status, loads hauled, drivers experience and work record, which include law violations, "you owe" amounts, vehicle abandonment, and return of legalization owned by this company.
